Release for Participation in Girls Making Media Workshops
Girls Making Media is a program in which participants create and upload media to: (includes but not limited to) blogs, www.myspace.com, www.facebook, and our www.youtube.com channel.
Please check each that is appropriate:
_________I give the right to use my (or my child’s) name, image, creative works (writing, photo, audio and video content created, produced, uploaded in the 12 hour workshop) to Girls Making Media and WomanKraft Art Center.
_________I grant Girls Making Media (GMM) permission to modify created work to suit media requirements or future compilations.
_________I, or my child, retain the right to the credit listed, and the ability to link to created media that I/my child helped create. Girls Making Media retains the copyright of the created media.
_________I grant my child permission to search keywords, research ideas and stories on the world wide web. All caution is taken to keep clear of inappropriate material.
________ I grant GMM permission to indefinitely share, forward, promote created media in social networks in the web.
_______ I grant GMM permission to submit created media to contests and video fests.
_______I waive any right to royalties or compensation for my participation in GMM, or for media created in GMM, unless stipend is stated at the beginning of session, and agreed to by both parties.
_______For parents/guardians: I understand that media is created by teens. I release WomanKraft and Girls Making Media from any liability if I don’t agree with the content.
_______Girls Making Media provides participants the use of computers, cameras, audio and video recorders in the course of media production workshops. If I, or my child, damage any piece of equipment, even by accident, I agree that it is my responsibility to replace the damaged item with one of equal quality/specifications.
Girls Making Media agrees to protect the privacy of each participant. No personal information will ever be given to anyone outside of program and only first names will be used in created media.
